Diamond Creek Disability & Health Care’s assistance with Accommodation and Tenancy services include all support required to overcoming barriers impacting current tenancy or support in exploring new alternative accommodation. Our staff are highly skilled and are able to provide reliable, innovative and flexible support to suit individual needs and goals. We understand caring for a person with disability can be hard at times. A break is just what is needed at times and we get that. Let us share your responsibility while you take that well-deserved break.
Our Tenancy service includes an assessment and provides solutions depending on an individual’s needs.

Diamond Creek Disability & Health Care provides NDIS Services in Melbourne. We take person centered, evidence-based approach to any services that we provide where the participant and their informal supports are primary to any decisions made. Diamond Creek Disability & Health Care exists to work with our participants, their advocates, family members and other service providers as relevant, to provide the services to meet our participants’ needs within the scope of our services.

Assist-Life Stage, Transition
Our experienced staff can work with participants and their families to prepare schedules for daily, weekly, and longer‐term support, ensuring that, at all stages, their supports are assisting them in the achievement of their goals. We can also assist with budgeting processes, including planning for expenses, making payments for one‐off purchases, and setting up payment arrangements for ongoing supports.

We at Diamond Creek Disability & Health Care, employ an excellent support staff that assists our participants with all daily personal tasks, and Our person-centric approach recognises that everybody is unique and thus our flexible plan is created to suit your goals and needs. You have the complete freedom to choose where you would prefer to access our services – in a community environment or in the comfort of your own home. You also have the choice to choose between short-term support or long-term care.
Our services include:

At Diamond Creek Disability & Health Care, we can assist you in gaining greater confidence and independence by providing effective solutions to your travel issues, we help you build your mobility so that you can achieve your daily goals and increase your independence.
we will work with you to develop a travel plan and assist you in achieving your travel goals. We provide effective public transport training and support staff who ensure that you are always safe and comfortable.
We increase your independence through public transport training by helping you learn public transport routes, bus/train timetables, buying, recharging, and using your MetroCard. Our goal is to help you get around and engage in your community independently. With this purpose in mind, we offer the option of private transport too. Whether you have to travel to school or work, to medical appointments, or for shopping, you can book our transport services.
The bookings must be scheduled in advance and can be used for:
- Medical appointments
- Transport to School or any educational facility
- Work
- Place of education
- Local shopping centre
- Visit to the local chemist
- Community events, groups or clubs
- Visiting a local library.
Community Nursing Care
We at Diamond Creek Disability & Health Care believe that community participation is an excellent way to socialize and learn new life skills. We strongly support you in participating fully in society. We focus on resilience, wellbeing, and personal growth so that you are able to successfully connect to the community. We support your participation in community, social or recreational activities, and in employment. We also provide assistance with transport to and from an activity. We also provide assistance with communication, personal care etc., while you are participating in or attending events.
Our services are tailored to suit your individual needs and goals so you can get the maximum support you need.

Daily Tasks/Shared Living
Diamond Creek Disability & Health Care employs specialised and highly trained staff who are experienced, dedicated, competent and caring so that you can live your life independently. Their specialised skills and personal capabilities help to empower you to complete your daily activities in your home and community. Our staff will supervise and/or assist you with daily life tasks that enhance your skills to live independently. We provide our service to each participant that resides in shared living.
Our support may include the following services:
- Assistance with Shopping
- Help with Budgeting and Money management
- Assistance with household activities like meal preparation, paying rent on time, household tasks etc
- Assistance with written communication, completion of forms and payment of bills
- Managing appointments

Innov Community Participation
At Diamond Creek Disability & Health Care, we offer our participants the opportunity for Innovative Community Participation that is designed specifically to support people with disabilities to learn new skills and polish pre-existing skills so that they can actively participate in the community and related events and programmers.
Our experienced, dedicated and respectful direct support staff and exceptional team is dedicated to your cause and helps you gain confidence in every step you take. The following services are included in our Innovative Community Participation program:
- Workshops to learn new skills
- Assistance with finding suitable employment: we assist you in building up on your skills so that you can effectively and independently find gainful employment

Development-Life Skills
Our Life-Skills Development Program focuses on training and developing your skills so that you can live as independently as possible. Everything from personal hygiene to grocery shopping and meal planning. We provide a range of support services to help you live as independently as possible Our training ranges from daily life skills, communication and social skills to problem solving and management of funds of supports. You can access training individually, or as a group activity.

Group/Centre Activities
Our Group or Centre-based activities program is designed to meet the needs of individuals and promote personal and social growth. This program is ideal for someone who wants to improve their quality of life by meeting new people, making new friends, and participating in activities. Our team members are fully qualified, professional, experienced and ready to set your goals with you and work with you to achieve it.
We encourage our participants to meet new people, make new friends, participate in activities, and learn new skills, all while socialising and having fun. Diamond Creek Disability & Health Care is a NDIS service provider in Melbourne.
Program activities include:
- Accessing community – library, art gallery, museum, shopping and attending community events.
- Life skills development – cooking classes, grocery shopping, personal care and household tasks.
- Recreational – bowling, music, picnics, arts and crafts
- Literacy and Numeracy – reading and writing skills, money handling and budgeting,
- Community access – eg: library, music works, community events, shopping, museum, art gallery, volunteer groups.
- Computer Literacy: This includes using computers, and resume building.
Improved Health & Wellbeing – Dance classes, group fitness activities, walks, swimming and yoga/gym
Implementing Behaviour Support Plans
Behaviour support plans attempt to lessen and eventually do away with the use of restrictive practises by developing alternative methods of behaviour management. They are designed to assist caregivers and providers in understanding and managing complex behaviours in a positive way.
Any use of restrictive practises must comply with the NDIS Restrictive Practices and Behaviour Support Rules, which state that they must be:
- Used as a last option where there is a risk of damage to the person or others
- Authorized in accordance with State or Territory laws or requirements for policy
- Based on the likelihood of injury
- Used only briefly whenever possible
The National Disability Insurance Agency is in charge of overseeing it (NDIA).
Before employing restrictive methods, providers must have the appropriate authorisation and be aware of any relevant State and Territory statutory or policy requirements.
A reportable occurrence is when an NDIS provider uses a restrictive practise without authorisation.
If a provider learns that a member of their organisation has engaged in restricted practise, they are required to notify the NSQC within five business days because it is a reportable offence.
